Curcumin: a targeted anti-inflammatory for endometriosis

Curcumin has been shown in several studies in vitro and animals:

• Inhibition of NF-kB in endometriotic cells (reduction of TNF-α, IL-6 by 40-60%)
• Pelvic aromatase inhibition → ↓ local estrogens
• Reduced proliferation of ectopic endometrial cells
• Inhibition of angiogenesis (VEGF) → less neovascularization of lesions

The problem : curcumin alone has an oral bioavailability <1%. Without a co-factor, the clinical effect is minimal.

Piperine: an essential multiplier

The piperine of black pepper multiply by 20× the bioavailability of curcumin by blocking glucuronidation liver-hepatic-protection-nash">hepatic (Shoba et al., 1998). This is the most rational combination: without piperine, curcumin is largely lost.

Ginger: complementary action

• Inhibits COX-2 → ↓ pro-painful prostaglandins
• Anti-nausea effect (useful during peri-menstrual periods)
• Antispasmodic effect on uterine smooth muscle

A pilot study (Phytotherapy Research, 2020) showed that 1g of ginger/day for 3 menstrual cycles reduces endometriosis pain by 30% (equivalent to an NSAID without the side effects gastric).

FAQs

Can INTI replace my NSAIDs during periods?

For moderate pain: possibly, after testing. For severe pain or patients with stage 3-4 endometriosis: no, keep the NSAIDs on prescription.

Can organic erythritol cause intestinal upset during periods?

Erythritol is very well tolerated (90% absorbed then excreted). At doses in INTI, no laxative effect expected.
