Pregnancy Nausea: The Role of 5-HT3
Pregnancy nausea affects 70–80% of pregnant women, especially in the 1st trimester:
- High hCG → stimulation of chemoreceptor trigger zone → 5-HT3 activation
- Estrogens → sensitization of 5-HT3 receptors in the gastrointestinal tract
- Progesterone → delayed gastric emptying → distension → nausea
- Hypersensitized vagus nerve → acid reflux → increased gag reflexes
Mechanism of Action of Ginger in Pregnancy Nausea
6-Shogaol and 6-gingerol bind as competitive antagonists to 5-HT3 receptors in the chemoreceptor trigger zone and gastrointestinal tract — the same mechanism as ondansetron (Zofran). Ginger also accelerates progesterone-delayed gastric emptying (+15–25% speed) → less distension → less nausea.
Clinical Studies
| Study | Participants | Dose | Result |
|---|---|---|---|
| Vutyavanich 2001 (RCT) | 70 women T1 | 1g/d (4×250mg) | Nausea −35%, vomiting −40% |
| Ensiyeh 2009 (RCT) | 67 women T1 | 1g/d vs metoclopramide | Comparable effectiveness |
| Meta-analysis Viljoen 2014 | 1278 women (12 RCTs) | 1–1.5g/d | Nausea −56% (NVP score) |
| Ozgoli 2009 (RCT) | 67 women | 1g/d vs vitamin B6 | Superior to vitamin B6 |
Safety in the First Trimester
- ✅ 1g/day: safety consensus in RCTs (T1, T2, T3)
- ✅ No increased congenital malformations in available studies (Cochrane 2016)
- ⚠️ >2g/day: insufficient data — caution advised
- ⚠️ History of miscarriage: consult gynecologist before use
- ⚠️ Anticoagulants: ginger is slightly anti-aggregant — monitor

❓ FAQ — Ginger and Pregnancy Nausea
From what week should I start taking ginger?
From the onset of nausea (often week 6–8). Maximum effectiveness documented in the 1st trimester.
Does ginger also work for hyperemesis gravidarum?
For severe hyperemesis (requiring hospitalization), data is insufficient. Medical treatment is then necessary. Ginger is effective for mild to moderate nausea.
Can ginger be combined with vitamin B6?
Yes — the mechanisms are different and complementary. This is often the combination initially recommended in various obstetric guidelines.
