Back pain: types and inflammatory mechanisms
80% of adults suffer from lower back pain at least once in their lives. The main types:
- Mechanical lower back pain: Muscle contractures, herniated disc, vertebral osteoarthritis
- Inflammatory lower back pain: Ankylosing spondylitis, facet joint arthritis

Ginger's mechanisms on back pain
1. COX-2 and prostaglandin inhibition
COX-2 produces prostaglandin E2 which sensitizes muscular and disc nociceptors. Ginger inhibits it with similar potency to ibuprofen for moderate pain, without the gastric side effects.
2. Reduction of muscle spasms
By reducing prostaglandins and IL-6 in muscle tissue, ginger attenuates reflexive contractures that accompany back pain.
3. Neuroprotection for sciatica
Preclinical studies show that ginger protects nerve fibers from ginger stress oxidative stress, potentially beneficial in sciatica related to a herniated disc.
Wigler 2003 Study: Ginger vs. Spinal Pain
Randomized crossover study on 29 patients with chronic pain (spinal osteoarthritis + lower back pain):
- 255mg ginger extract twice daily × 8 weeks
- Result: pain -33%, mobility +20%, 75% patient satisfaction
- No significant gastric effects vs NSAIDs
