Study : 44 patients with NASH, randomized into 2 groups. Treatment group: 2g/day of ginger extract for 12 weeks. Control group: placebo.

Results :

  • Significant reduction in ALT and AST (liver enzymes) in the ginger group
  • Improvement in insulin resistance (score ginger HOMA-IR -28%)
  • Reduction in NASH activity score (NAS score)
  • Decreased hepatic resistin and adiponectin

Hepatoprotective molecular mechanisms

  • PPAR-α activation : [6]-gingerol activates the peroxisome proliferator-activated receptor alpha, which stimulates the beta-oxidation of fatty acids in hepatic mitochondria
  • SREBP-1c inhibition : reduces de novo lipogenesis (fat synthesis by the liver)
  • Induction of antioxidant enzymes : SOD, catalase, glutathione peroxidase
  • Hepatic TNF-α reduction : decrease in hepatocyte apoptosis

Ginger and liver detoxification

The liver detoxifies via 2 enzymatic phases:

  • Phase I (cytochromes P450): biotransformation of toxins
  • Phase II (glutathione-S-transferases): conjugation and elimination

Ginger induces phase II enzymes, notably glutathione-S-transferases, increasing hepatic detoxification capacity (Young et al., 2006, Phytomedicine).
