Endometriosis lesions overexpress aromatase (CYP19A1), converting androgens into estradiol locally. This mechanism creates a local estrogen dominance independent of the ovaries. PGE2 stimulates aromatase, aromatase produces estradiol, estradiol stimulates COX-2 → PGE2 → vicious cycle.
