Endometriosis in Belgium: The Underdiagnosed Women's Disease
Endometriosis is a chronic gynecological condition in which tissue similar to the lining of the uterus grows outside the uterus — on ovaries, fallopian tubes, peritoneum, and sometimes other organs. It is fueled by estrogens.
- Affects ~10-15% of Belgian women of reproductive age (~1 million)
- Average diagnostic delay: 7-9 years
- Characteristics: dysmenorrhea (severe menstrual pain), dyspareunia, infertility, chronic pelvic pain
Sugary Drinks and Endometriosis
Insulin → Aromatase → Estrogen Dominance
Sugary drinks increase insulin, which activates aromatase (CYP19A1) in peritoneal endometrial tissue and fat cells. Locally produced estrogen stimulates the proliferation of endometriosis lesions and prolongs the menstrual cycle — a vicious cycle of estrogen dominance that directly feeds endometriosis.
NF-κB → PGE2 → Severe Menstrual Pain
Endometriosis lesions already produce elevated amounts of COX-2 and PGE2 (prostaglandin E2), leading to severe cramping pain. Sugar activates NF-κB in peritoneal cells, increases COX-2 expression, and exacerbates PGE2 production — intensifying the symptoms of dysmenorrhea.
Fructose → AGE → Peritoneal Glycation
Fructose from soft drinks increases the formation of AGEs (advanced glycation end-products) in peritoneal tissues. AGEs activate RAGE on endometriosis lesions, stimulate VEGF (angiogenesis → lesion size increase), and worsen adhesion formation.
| Drink | Sugar/100 ml | Aromatase effect | Endometriosis impact |
|---|---|---|---|
| Coca-Cola | 10.6 g | ❌ Aromatase ↑↑ | ❌ Lesion growth ↑ pain ↑ |
| Innocent Smoothie | ~12 g | ❌ High fructose | ❌ AGE ↑ |
| Lipton Ice Tea | 8.8 g | ❌ Insulin ↑ aromatase ↑ | ❌ PGE2 ↑ |
| GIMBER diluted (4 cl/200 ml) | ~7 g | ⚠️ ginger shot without sugar + cane sugar | ⚠️ Partially |
| INTI diluted (4 cl/200 ml) | <2 g | ✅ 6-gingerol inhibits aromatase | ✅ COX-2 ↓ PGE2 ↓ NF-κB ↓ |
INTI in Endometriosis: Active Mechanisms
6-gingerol: COX-2/PGE2 Inhibition (Pain Relief)
6-gingerol inhibits COX-2 and PGE2 production in endometriosis lesions, reducing prostaglandin-mediated pain (dysmenorrhea). A randomized clinical trial (Iran. J. Nurs. Midwifery Res., 2014) shows significant pain reduction in women with primary dysmenorrhea after ginger supplementation. Endometriosis-associated pain shares the same COX-2/PGE2 pathway.
Aromatase Inhibitory Effect
In vitro studies show that 6-gingerol and 6-shogaol inhibit aromatase (CYP19A1) in endometriosis lesions, thus decreasing local estrogen production — similar (but weaker) to letrozole (an aromatase inhibitor used in endometriosis).
Curcumin: CYP1A1 and Estrogen Catabolism
Curcumin induces CYP1A1 in the liver, which stimulates estradiol-2-hydroxylation (protective pathway) and reduces estradiol-16α-hydroxylation (tumorigenic pathway) — better estrogen balance for endometriosis management.
❓ Frequently Asked Questions — INTI and Endometriosis
Can INTI replace hormonal therapy for endometriosis?
No. Hormonal therapies (progestins, GnRH agonists, aromatase inhibitors) are the medical standard for endometriosis. INTI is a dietary supplement to support a low-sugar, anti-inflammatory diet.
Is ginger safe during menstruation with endometriosis?
Yes. Ginger has antiemetic and antispasmodic properties that can also relieve abdominal pain and nausea during menstruation. At nutritional doses, no negative effects on menstrual pain have been reported.
INTI contains black pepper — does this increase estrogen levels?
No. Piperine has no phytoestrogenic effect. However, it increases curcumin bioavailability (×20), which enhances the anti-inflammatory-science-utilisation">ginger anti-inflammatory curcumin effect.
INTI: less than 4g of sugars/100ml. Ginger (COX-2 ↓ aromatase ↓) + turmeric (CYP1A1 ↑) organic. On inti-drink.com and in pharmacies in Belgium.
