Step 4: Urate crystals and NLRP3 inflammation
When uric acid levels exceed 6.8 mg/dL, monosodium urate (MSU) crystals precipitate in the joints. These crystals activate the NLRP3 inflammasome in macrophages, with massive release of IL-1β — responsible for the characteristic severe pain of a gout attack (VAS 8-10/10).
