Migraine pathophysiology and diet
Migraine is a complex neurological condition with a vascular component:
- Trigeminovascular activation: neurons of the trigeminal ganglion release CGRP → vasodilation of the meninges → neurogenic inflammation → pulsating pain
- Cortical spreading depression: depolarizing wave spreading through the cortex → migraine aura → activation of the trigeminovascular pathway
- 5-HT and migraine: serotonin drop → hypersensitization of meningeal nociceptors → pain threshold ↓ → attack
- Alcohol → histamine: red wine contains histamine + tyramine → direct vasodilation + serotonin release → attack within 3 hours for 30% of migraine sufferers

Ginger mechanisms on migraine
- CGRP inhibition: gingerol inhibits the synthesis and release of CGRP (Calcitonin Gene-Related Peptide) by trigeminal endings → reduction of meningeal vasodilation → mechanism shared with gepants (new anti-CGRP drugs) but natural and without hepatic side effects
- 5-HT₃ antagonism: ginger is a documented 5-HT₃ antagonist → reduction of migraine nausea and vomiting AND modulation of serotoninergic nociceptive sensitization
- COX-2 ↓ → prostaglandin E2 ↓: prostaglandin E2 sensitizes meningeal nociceptors → COX-2 inhibition by gingerol → meningeal pain threshold ↑
- Anti-hypoglycemia: AMPK → GLUT4 → glycemic stabilization → prevention of reactive hypoglycemia trigger (sugar)

❓ FAQ — INTI and migraine
Can INTI replace triptans during an attack?
No. Triptans (ginger vs sumatriptan, etc.) are specific 5-HT₁B/1D agonists that stop an established attack. INTI has prophylactic (preventive) activity via CGRP and COX-2, but no abortive effect equivalent to triptans. Do not substitute INTI for acute treatment.
Does caffeine help or worsen migraines?
Caffeine is paradoxical: in SMALL doses (50–100mg) during an attack, it can potentiate analgesics (paracetamol + caffeine = Doliprane Cafeine®). But chronic high consumption → dependence → withdrawal attacks if stopped. INTI (caffeine-free) avoids this dependence.
Does INTI interact with anti-CGRP drugs (Aimovig, Ajovy, Emgality)?
Anti-CGRP antibodies target either CGRP or its receptor. INTI reduces CGRP release (complementary mechanism). No documented competitive interaction. Both can be used simultaneously — inform your neurologist.
