SIBO in Belgium: an underdiagnosed intestinal condition
SIBO is the excessive growth of bacteria in the small intestine (>10⁵ CFU/ml), where normally few bacteria are present. It leads to the fermentation of carbohydrates in the small intestine, producing gas (H₂, CH₄, H₂S) and symptoms such as:
- Bloating and abdominal pain (often 1-2 hours after a meal)
- Alternating bowel movements (diarrhea or constipation)
- Malabsorption (fats, B12, iron)
- Fatigue and brain fog (due to endotoxin absorption)
How sugar feeds and worsens SIBO
Fermentable sugars → bacterial growth stimulation in the small intestine
Sugary drinks contain glucose, fructose, and sucrose, which are readily available as substrates for bacteria in the small intestine. In SIBO, these sugars ferment in the small intestine (before reaching the large intestine), causing gas, abdominal pain, and distension. Every Coca-Cola (35 g of sugar) provides food for bacteria before carbohydrates are normally absorbed.
Fructose → increased hydrogen production in SIBO
Fructose malabsorption is particularly prevalent in SIBO patients (small intestine with reduced GLUT5 capacity). Fructose from soft drinks and fruit juice reaches the overgrowing bacteria and stimulates H₂ production (measurable via breath H₂ test), worsening SIBO symptoms.
Sugar → reduced gut motility (MMC)
A sugar-rich diet slows down the migrating motor complex (MMC) — the "cleansing waves" that flush bacteria from the small intestine between meals. MMC disruption is a central cause of SIBO. Sugar-induced slowing of the MMC promotes bacterial stasis and SIBO recurrence after treatment.

Ginger and SIBO: prokinetic and antibacterial action
Prokinetic action: 5-HT4 and motilin
Ginger extracts stimulate gut motility via 5-HT4 serotonin receptors (like metoclopramide, a pharmacological prokinetic) and motilin receptors. This accelerates gastric emptying and small intestinal transit, stimulates the migrating motor complex (MMC), and flushes bacteria towards the large intestine — the core goal of SIBO treatment.
Antibacterial: inhibition of bacterial overgrowth
6-gingerol and 10-gingerol possess antibacterial properties against the bacterial species typically causing SIBO (Escherichia coli, Klebsiella, Streptococcus). They also inhibit the formation of bacterial biofilms in the small intestine.

Can INTI treat SIBO with antibiotics?
No. Rifaximin (antibiotic) or neomycin are the proven SIBO treatments. INTI can support as a prokinetic to reduce SIBO recurrence but does not replace antibiotic treatment.
Is INTI FODMAP-friendly for SIBO diet?
INTI contains no polyols, fructans, or GOS. It is low-FODMAP. Ginger itself is FODMAP-friendly in normal portions. Diluted INTI fits into a low-FODMAP SIBO diet.
Can ginger worsen SIBO?
Not at normal doses. Ginger as a prokinetic promotes bacterial clearance. Some SIBO patients are sensitive to fructans (present in onion/garlic, NOT in ginger). Ginger itself is excellently tolerated in SIBO.
