IL-6 (Interleukin-6): the acute phase and PMR cytokine
IL-6: Interleukin-6
IL-6 (Interleukin-6) is a pleiotropic cytokine, central to the acute phase response (CRP, fibrinogen, ferritin). It is a major player in polymyalgia rheumatica (PMR), giant cell arteritis (Horton's disease), severe COVID, and numerous inflammatory conditions. It is the therapeutic target of tocilizumab (anti-IL-6R).
Characteristics
- gp130 family cytokine
- Receptor: IL-6R + glycoprotein 130 (gp130)
- Production: macrophages, T cells, fibroblasts, adipocytes
- Signaling pathway: JAK1/JAK2 → STAT3
Biological Role
- Acute phase response: induces CRP, fibrinogen, hepcidin, ferritin at the hepatic level
- Differentiation of Th17 → maintenance of autoimmune diseases
- Osteoclast activation → inflammatory osteoporosis
- Chronic inflammation → cachexia, sarcopenia
Diseases with elevated IL-6
- Polymyalgia rheumatica (PMR) — IL-6 is the central marker
- Horton's disease (giant cell arteritis)
- Rheumatoid arthritis
- Castleman disease
- Severe COVID-19 ("cytokine storm")
- Cancer cachexia
Natural Inhibitors
- [6]-gingerol: ↓ IL-6 by 40-60% in activated macrophages
- Curcumin: directly inhibits the JAK/STAT3 pathway
- Piperine: potentiates curcumin
- Resveratrol, omega-3 EPA/DHA
IL-6 and INTI
IL-6 inhibition is one of the most documented effects of ginger + turmeric. INTI targets this pathway through several complementary mechanisms.
