1. Interstitial Cystitis: Chronic Bladder Neuroinflammation
IC/PBS affects 3–8% of women. Mechanisms:

2. Ginger Mechanisms in IC
2.1 NF-κB → IL-6/IL-8/NGF in urothelium
6-Gingerol reduces NF-κB in human urothelial cells: IL-6 −44%, IL-8 −39%, NGF −48%. NGF reduction is crucial: NGF is the primary sensitizer of bladder C-fibers → less NGF → less hypersensitization → less pain with bladder filling.
2.2 TRPV1 desensitization in bladder
6-Gingerol activates and desensitizes TRPV1 in bladder C-fibers (analogous to intravesical slimming-thermogenesis-weight-loss-shot">capsaicin, but oral). Result: increased pain threshold with bladder filling, reduced voiding frequency in animal models.
2.3 Mast cell degranulation inhibition
Reduced histamine and PGE2 → less submucosal inflammation → improved bladder comfort.
2.4 COX-2/PGE2 analgesia in pelvis
COX-2 inhibition in bladder mucosa → PGE2 −35–45% → less activation of EP1/EP3 receptors on afferent fibers → reduction of chronic pelvic pain.
