Eczema: A Th2-mediated inflammation of the skin
Atopic dermatitis (eczema) is a chronic inflammatory skin condition characterized by:
- Filaggrin deficiency (skin barrier protein)
- Dominant Th2 immune response (IL-4, IL-13, IL-31)
- Mast cell activation (histamine → pruritus)
- Cutaneous dysbiosis (dominant Staphylococcus aureus)
Mechanisms of ginger on eczema
1. Inhibition of Th2 cytokines
Gingerols inhibit the production of IL-4 and IL-13, key cytokines for Th2 polarization in eczema. This mechanism is partly analogous to that of dupilumab (Dupixent), the anti-Th2 biologic for severe eczema.
2. Mast cell inhibition (antihistamine)
Ginger reduces mast cell degranulation, limiting the release of histamine and leukotrienes responsible for intense pruritus.
3. Antibacterial activity against S. aureus
S. aureus colonizes 90% of eczematous skin and amplifies inflammation. Gingerols inhibit S. aureus, reducing the skin's bacterial load.
4. Strengthening the skin barrier
Ginger stimulates the synthesis of ceramides and filaggrin — essential for the deficient skin barrier in eczema.
Topical vs. oral use for eczema
- Oral (INTI): 15-20ml/day — systemic anti-inflammatory ginger action on Th2 cytokines
- Topical (caution!): Highly diluted ginger (1% in neutral cream) — test on a small area first; pure ginger can irritate eczematous skin
- Absolutely avoid: Applying pure ginger to broken skin or during an acute flare-up
