Pathophysiology of Rheumatoid Arthritis
RA is an autoimmune disease (prevalence ~1% in Belgium) characterized by:
- Activation of autoreactive T lymphocytes → attack on synovial tissue
- Pannus formation (abnormal inflammatory tissue in the synovium)
- Production of pro-inflammatory cytokines (TNF-α, IL-1β, IL-6, IL-17)
- Progressive destruction of cartilage and subchondral bone
Mechanisms of Ginger in RA
1. Inhibition of Synovial Cytokines
6-gingerol and 6-shogaol inhibit NF-κB in synoviocytes (cells of the synovial membrane) → reduction of:
- TNF-α: -28% in vivo (adjuvant-induced arthritis model)
- IL-1β: -32% (cartilage destruction inducer)
- IL-17: -25% (orchestrator of anti-inflammatory-inflammation-natural-remedy">synovial inflammation)
- COX-2 and PGE2: -35–45% (pain prostaglandins)
2. Inhibition of Metalloproteinases (MMP-3)
MMP-3 (stromelysin) degrades turmeric-rides-peau-naturel-2026">type II collagen of articular cartilage. A study on RA synoviocytes: ginger reduces MMP-3 expression induced by IL-1β by -40% — cartilage protection.
3. Cumulative Clinical Data (2020 Meta-analysis)
Meta-analysis (Phytother. Res., 2020) on 5 RCTs, 344 RA patients:
- Pain reduction (VAS): -31% (95% CI: -40 to -22%)
- DAS28 score reduction (disease activity): -18%
- CRP reduction: -22%
Ginger and Disease-Modifying Antirheumatic Drugs (DMARDs)
DMARDs (methotrexate, leflunomide, hydroxychloroquine) are the cornerstone of RA treatment. Ginger can be complementary but:
- ⚠️ Methotrexate: Ginger does not affect MTX pharmacokinetics at dietary doses
- ⚠️ NSAIDs (ibuprofen, naproxen): Possible COX-2 potentiation — monitor for gastric symptoms
- ⚠️ Anticoagulants: Caution if on warfarin or new anticoagulants

Does ginger help with other inflammatory arthropathies (spondylitis, ginger lupus)?
The anti-NF-κB and anti-cytokine action is common to several inflammatory arthropathies. Similar benefits are likely for ankylosing spondylitis and some forms of lupus, but specific data are limited.
