🔬 Interstitial Cystitis and Irritating Beverages
1. Acidity: The Direct Enemy of the Urothelium
The urothelium — the bladder lining — is covered by a protective layer of glycosaminoglycans (GAGs): heparan sulfate, chondroitin sulfate, hyaluronic acid. In IC, this layer is damaged or deficient, exposing urothelial cells to irritating urinary constituents. Acidic drinks (pH <5):
- Lower urinary pH → directly irritating to the exposed urothelium
- Activate sub-mucosal mast cells → histamine release → vasodilation and pain
- Disrupt GAG synthesis → worsen the deficient protective layer
2. Caffeine: Double Bladder Irritant
Caffeine:
- Is excreted by the kidney as metabolites irritating to the urothelium (1-methylxanthine, 1-methylurate)
- Stimulates histamine release from bladder mast cells → inflammation
- Inhibits bladder adenosine A1 receptors → detrusor hyperactivity → urinary urgency
3. Sugars and Oxaluria

Anti-inflammatory Mast Cell Inhibition
6-gingerol inhibits mast cell degranulation by blocking the FcεRI pathway and reducing histamine and tryptase release. In IC, sub-mucosal mast cells are chronically activated — reducing their degranulation can decrease the frequency of flares.
Anti-nociceptive via TRPV1
TRPV1 is overexpressed in bladder afferent nerve fibers in IC patients — it lowers the visceral pain threshold. 6-Gingerol desensitizes TRPV1 → less bladder hyperalgesia → reduced pain during flares.
Anti-COX-2: Reduction of Bladder PGE2
PGE2 (prostaglandin E2) is the primary mediator of pain and urinary urgency in IC. COX-2 is overexpressed in the IC urothelium. 6-Gingerol inhibits COX-2 → less PGE2 → less pain and urgency.

Can INTI trigger an IC flare?
Pure (undiluted): possible — pH of ~4 can irritate. Very diluted (1 shot/300ml): well tolerated for the majority of IC patients based on feedback. Ginger itself is not on the Interstitial Cystitis Network's list of classic bladder irritants.
Which acids in INTI could be problematic?
Citric acid from lemon (present in small amounts when diluted) is the main candidate for an irritant in IC. Some IC patients are sensitive to citrus. Strong dilution significantly reduces this risk.
Is alkaline water better for IC?
Many IC patients report relief with alkaline water (pH 8–9) — it slightly alkalizes urine, reducing urothelial irritation. INTI + alkaline water (rather than still) can maximize compatibility.
