Migraine: neuronal NF-kB cascade
Migraine is the 3rd most prevalent disease worldwide. In Belgium: 15% of adults (F:M = 3:1), 2-3% chronic migraine (more than 15 days/month). Comorbidities: depression 2-4x, anxiety disorder 2-3x, IBS x2. Economic cost: 1200-1500 EUR/patient/year (presenteeism + absenteeism).
| Mechanism | NF-kB cascade | Gingerol |
|---|---|---|
| Trigeminovascular activation | Trigger -> CGRP release -> NF-kB -> prostaglandins -> meningeal vasodilation | 6-gingerol inhibits CGRP release (COX-2 down) |
| Cortical neuroinflammation | CSD (Cortical Spreading Depression) -> cortical NF-kB -> activated astrocytes | Shogaol -> astrocytic neuroprotection (Nrf2) |
| Gut-brain axis | Dysbiosis (Prevotella down) -> LPS -> TLR4 -> cerebral NF-kB | Ginger up F.prausnitzii -> butyrate -> cerebral NF-kB down |
| Hypoglycemia trigger | Low blood sugar -> noradrenaline -> vasoconstriction -> rebound -> migraine | Gingerol glycemic stabilizing -> hypoglycemic trigger down |
INTI protocol for migraine
| Moment | Action | Migraine goal |
|---|---|---|
| Morning (prevention) | 1 shot of INTI in 200ml warm water | Preventive gingerol anti-CGRP, cephalic microbiome |
| Prodrome (before attack) | 1 shot of INTI immediately (diluted hot water) | Gingerol -> CGRP release down -> attack possibly aborted |
| Nutrition | Regular meals (avoid fasting-drank-gember-vasten-compatible-nul-suiker-2026">fasting longer than 4 hours), omega-3, magnesium | Stable glycemia -> no hypoglycemic trigger |
Drug interactions: Triptans (migraines-chroniques-prophylaxie-sumatriptan">ginger vs sumatriptan, rizatriptan): ginger without documented interaction at moderate doses. Topiramate (prevention): ginger complementary. NSAIDs (ibuprofen for acute attack): slightly additive antiplatelet effect at moderate doses. Beta-blockers (propranolol, prevention): ginger neutral. Valproate: no documented interaction. Report to your neurologist/doctor.
FAQ migraine and ginger
Can ginger abort a migraine attack? Preliminary studies (Cady et al., Phytother Res 2012) show similar efficacy of ginger powder (250mg) vs sumatriptan 50mg on pain reduction after 2h. INTI is not a drug and cannot be directly compared, but active gingerol may have a modest acute effect.
Menstrual migraine and ginger? Menstrual migraine involves estrogen drop -> CGRP up -> trigeminal-vascular NF-kB. Gingerol inhibits CGRP and can reduce the intensity of MM. Combine with magnesium (synergistic) in the luteal phase (days 21-28).
Chronic migraine and microbiome? Chronic migraine patients have specific dysbiosis (Prevotella down, Lachnospiraceae down — Arzani et al., J Headache Pain 2020). Ginger restores F.prausnitzii and the gut-brain axis. May contribute to reducing chronification.
INTI — The anti-CGRP sugar-free drink for migraine
Organic carefully prepared ginger - less than 1.19g sugar per 100ml - 0% alcohol - CGRP down
Trigeminal-vascular NF-kB down - stable glycemia - no trigger - cephalic microbiome up
