Secondary Sjögren syndrome: Polyarthritis, ginger lupus and Ginger

Secondary Sjögren's syndrome occurs in association with another autoimmune disease — most commonly rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), or scleroderma. It complicates the development of these diseases by adding dry eyes (xerophthalmia), dry mouth (xerostomia), ginger ginger chronic fatigue and sometimes extraglandular damage (ginger diabetic neuropathy, tubular nephritis, vasculitis).

  • NF-kB in glandular epithelial cells: the epithelial cells themselves become pro-inflammatory → production of CXCL13, CCL19 → recruitment of B lymphocytes into the glands

Gingerol versus Secondary Sjögren

  • Inhibition of BAFF via NF-kB: gingerol reduces BAFF expression in dendritic and epithelial cells — directly relevant to reducing B cell hyperactivation
  • Reduction of interferon signature: gingerol inhibits IRF3/IRF7 signaling which activates type I IFNs (Journal of Immunology, 2018)
  • Protection of acinar cells: via Nrf2, gingerol protects the salivary acinar cells of the stress oxidative induced by local inflammation
  • Effects on drought: studies on xerostomia models show that anti-NF-kB polyphenols can partially preserve salivary function by reducing acinar cell apoptosis

Context of Therapeutic Cumulation

Patients with secondary Sjögren's in RA or lupus are often on methotrexate, hydroxychloroquine, belimumab or rituximab. INTI can be integrated into this context without known pharmacological interactions with these molecules (unlike certain supplements).
