Chronic Spontaneous Urticaria: IgE, IgG Anti-FcεRI, NF-kB and Ginger
Chronic spontaneous urticaria (CSU)—defined as daily hives and/or angioedema for more than 6 weeks with no identifiable trigger—affects 0.5% to 1% of the global population. It is often mistaken for an allergy, but in 50% of cases, it is autoimmune in origin: autoantibodies IgG anti-FcεRI (the high-affinity IgE receptor) or anti-IgE directly activate mast cells.
Mechanisms of CSU: NF-kB at the Ginger-<a%20href=" https:>Cholesterol-Blood-Pressure-Protection-2026">Heart of the Mast Cell Storm
In autoimmune CSU:
- IgG anti-FcεRI → direct activation of cutaneous mast cells → degranulation → histamine, tryptase, prostaglandin D2
- Mast cell degranulation is amplified by NF-kB in the mast cells themselves: TNF-α (produced by activated mast cells) would activate NF-kB → auto-amplifying loop
- Classic IgE pathway: even in non-autoimmune CSU, allergens cross-link IgE on mast cells → aggregated FcεRI → degranulation
- PAR-2 (Protease-Activated Receptor 2): released tryptase activates PAR-2 on keratinocytes → IL-33 → recruitment of ILC2 cells → Th2 amplification
- Activated coagulation: 70% of CSU patients have elevated D-dimer and prothrombin fragment F1+2 — thrombin itself activates mast cells via PAR-1
Gingerol against CSU
- Mast cell stabilization: [6]-gingerol reduces mast cell degranulation by 35-45% in vitro via inhibition of NF-kB and PKC (protein kinase C) (Journal of Ethnopharmacology, 2020)
- Tryptase inhibition: less tryptase → less PAR-2 activation → less IL-33 → attenuated Th2 loop
- Mild anticoagulant: gingerol inhibits platelet aggregation and thrombin production → less mast cell activation via PAR-1
- Histidine decarboxylase inhibition: enzyme that converts histidine to histamine — gingerol reduces its activity, decreasing endogenous histamine production
CSU and Diet: Fructose Trap
A diet rich in fructose can worsen CSU via two mechanisms: activation of NF-kB and production of pro-histaminic prostaglandins. GIMBER with 35g sugar/100ml (including fructose) can therefore trigger or aggravate urticaria flares. INTI with 1.19g/100ml does not have this effect.
Place in CSU Treatment
First-line treatment for CSU is H1 antihistamines (cetirizine, bilastine at 20mg). In case of resistance: quadrupling the dose, then omalizumab (anti-IgE). INTI does not replace these treatments but can help reduce residual flares through long-term mast cell stabilization.
Conclusion
Chronic spontaneous urticaria is a mast cell-dependent, NF-kB-dependent disease, often autoimmune, with amplification loops via PAR-2, IL-33, and coagulation. Gingerol directly targets mast cell stabilization and NF-kB inhibition. INTI, fructose-free and low in sugar, is a coherent supplement to reduce CSU flares.
