TNF-α (Tumor Necrosis Factor alpha) is a major pro-inflammatory cytokine produced primarily by activated macrophages. It plays a central role in rheumatoid arthritis, Crohn's disease, ankylosing spondylitis, and most chronic inflammatory diseases. It is the target of anti-TNF biotherapies (infliximab, adalimumab, etc.) — one of the most prescribed therapeutic classes in rheumatology/gastroenterology.

- Type 2 cytokine (transmembrane then cleaved by TACE/ADAM17)
- Receptors: TNFR1 (constitutive) and TNFR2 (inducible)
- Serum half-life: 5-15 minutes
- Production: macrophages, T lymphocytes, NK cells, mast cells, fibroblasts

- Activates NF-kB by binding to TNFR1 → amplified inflammatory cascade
- Induces apoptosis of target cells via caspase-8
- Stimulates the expression of COX-2, iNOS, MMPs
- Recruits neutrophils and monocytes into tissues
- Activates osteoclasts → bone resorption
