Acne: multifactorial follicular inflammation
Acne affects 80% of teenagers and 30% of adults in Belgium (including 50% of adult women with hormonal acne). Pathogenesis: excessive sebum production (androgens), follicular hyperkeratinization, proliferation of Cutibacterium acnes, and local inflammation via IL-1β, TNF-α, PGE2. Ginger acts on 3 of these 4 factors.
Mechanisms of ginger against acne
1. Antibacterial action against C. acnes
Cutibacterium acnes proliferates in clogged sebaceous follicles. Gingerols: MIC against C. acnes 0.3–1 mg/ml in vitro. 60–70% reduction in follicular colonization in ex-vivo models. Potential synergy with topical antibiotics to reduce resistance.
2. Inhibition of follicular COX-2 and inflammation

3. Sebum regulation via local androgens
Hormonal acne in women (perimenstrual, PCOS) is linked to free testosterone that stimulates androgenic sebaceous receptors. Ginger → free testosterone ↓28% (via reduction of hyperinsulinemia, documented in PCOS) → reduced androgenic sebum stimulation.
4. Anti-hyperpigmentation post-acne
Post-acne pigment spots (PIH) remain visible for weeks. Gingerols → tyrosinase inhibition (melanin enzyme) → reduction of residual hyperpigmentation.

Diluted ginger extracts show anti-C. acnes activity in the lab. Practically: mix a few drops of fresh ginger juice into your moisturizer (patch test first — can be irritating). No controlled clinical topical study has been published.
Does ginger also help with back acne or chest acne?
Yes, the same mechanisms apply. The internal approach of ginger is even more relevant for back acne (difficult to reach topically).
