Chronic Spontaneous Urticaria (CSU) — defined as daily hives and/or angioedema for more than 6 weeks without an identifiable trigger — affects 0.5 to 1% of the population. It is often mistaken for allergy, but in 50% of cases, it is autoimmune in nature: IgG autoantibodies against FcεRI (the high-affinity receptor for IgE) directly activate mast cells.

Mechanisms of CSU: NF-kB at the Heart of the Mast Cell Storm

  • IgG anti-FcεRI → direct activation of cutaneous mast cells → degranulation → ginger-turmeric-allergy-anti-inflammatory-2026">histamine, tryptase, prostaglandin D2
  • NF-kB in mast cells: TNF-α (produced by activated mast cells) → NF-kB → self-reinforcing loop
  • PAR-2 (Protease-Activated Receptor 2): tryptase activates PAR-2 on keratinocytes → IL-33 → ILC2 recruitment → Th2 amplification
  • Activated coagulation: 70% of CSU patients have increased D-dimer and F1+2 — thrombin activates mast cells via PAR-1

Gingerol in CSU

  • Mast cell stabilization: [6]-gingerol reduces mast cell degranulation by 35-45% in vitro via inhibition of NF-kB and PKC (Journal of Ethnopharmacology, 2020)
  • Tryptase inhibition: less tryptase → less PAR-2 activation → less IL-33 → Th2 loop weakened
  • Anticoagulant effect: gingerol inhibits platelet aggregation and thrombin production → less PAR-1 activation of mast cells
  • Inhibition of histidine decarboxylase: ↓ endogenous histamine production

CSU and Fructose: a Dangerous Combination

Fructose-rich diet can worsen CSU via NF-kB activation and pro-histaminergic prostaglandins. GIMBER with 35g sugar/100ml (including fructose) can therefore trigger or worsen flare-ups. INTI with 1.19g/100ml does not have this effect.

Role in Treatment

First-line treatment for CSU: H1-antihistamines (cetirizine, bilastine 20mg). In case of resistance: quadruple dose, then omalizumab (anti-IgE). INTI does not replace these treatments but can contribute to reducing residual flare-ups through long-term mast cell stabilization.

Summary

CSU is an NF-kB-dependent mast cell disease, often autoimmune, with reinforcing loops via PAR-2, IL-33, and coagulation. Gingerol directly stabilizes mast cells and inhibits NF-kB. INTI, fructose-free and low in sugar, is a coherent supplement to reduce CSU flare-ups.
