Behçet's disease is a systemic vasculitis affecting vessels of all calibers—capillaries, veins, and arteries. Characterized by the classic triad: recurrent oral aphthae, genital aphthae, and uveitis.   • Th1/Th17 imbalance: excess IFN-γ (Th1) and IL-17A (Th17) → vascular inflammation
  • Increased IL-18 → NF-kB in neutrophils and macrophages → cascade of IL-1β and IL-6
  • Treg deficiency: regulatory T lymphocytes insufficient to counterbalance Th17 inflammation
  • HLA-B51: strongest genetic risk factor (OR ~6)

Gingerol in Behçet's: Relevant Mechanisms

  • Inhibition of neutrophilic NF-kB: ↓ oxidative burst, ↓ NETosis → reduced endothelial vascular damage
  • Anti-Th17: ↓ IL-17A via RORγt and STAT3 inhibition → particularly relevant as IL-17 is central in Behçet's
  • IL-18 suppression: gingerol lowers IL-18 in activated macrophages — key cytokine in Behçet's flares
  • Effect on oral mucosa: topical ginger applications showed ↓ duration and intensity of aphthae in pilot studies
  • Neuroprotection in neuro-Behçet's: ↓ neuroinflammation via microglial NF-kB inhibition

Treatment and Role of INTI

Behçet's treatment depends on its manifestations: colchicine for aphthae and arthritis, azathioprine or infliximab for uveitis, anticoagulation for thrombosis.
