ANCA-Vasculitis: Wegener's Granulomatosis, NF-kB, and Ginger

ANCA-Vasculitis: Wegener's Granulomatosis, NF-kB and ginger shot without sugar without sugar

ANCA-associated vasculitides (granulomatosis with polyangiitis [GPA/Wegener], microscopic polyangiitis [MPA], and eosinophilic granulomatosis with polyangiitis [EGPA/Churg-Strauss]) are rare but potentially life-threatening diseases affecting the kidneys, lungs, and nervous system.

Mechanisms in ANCA-Vasculitis: NF-kB as a Central Player

  1. Neutrophils primed by cytokines (TNF-α, IL-18) express PR3 and MPO on their surface
  2. ANCAs bind to these antigens → neutrophil activation → NETosis (NETs with DNA, PR3/MPO)
  3. NETs activate endothelial cells → NF-kB → adhesion molecules → neutrophil recruitment in the vessel wall
  4. Perivascular granulomas → tissue destruction (kidneys, lungs, ENT, nerves)
  5. IL-17A (Th17): enhances neutrophil recruitment and ANCA production

Gingerol in ANCA-Vasculitis: Relevant Targets

  • Inhibition of NETosis: gingerol reduces NET release by activated neutrophils (anti-inflammatory-science-utilisation">ginger-turmeric-black-pepper-chronic-pain">natural anti-inflammatory Research, 2021)
  • Inhibition of endothelial NF-kB: ↓ VCAM-1, ICAM-1, E-selectin → reduced neutrophil recruitment
  • Anti-Th17: ↓ IL-17A via RORγt and STAT3 inhibition
  • ↓ TNF-α (40-70%): TNF-α is the cytokine that primes neutrophils for ANCA activation

Essential Clinical Position

ANCA-vasculitides are life-threatening diseases. Induction treatment: rituximab or cyclophosphamide + corticosteroids. INTI is not a treatment for ANCA-vasculitis — it is at most a modest adjuvant in the maintenance phase. Extreme caution is advised: patients on immunosuppressants must discuss any supplement with their nephrologist/rheumatologist.

Practical Note

ANCA patients in maintenance on azathioprine or rituximab often have metabolic restrictions. INTI with 1.19g sugar/100ml and an anti-inflammatory profile is compatible with most protocols, provided medical validation. GIMBER with 35g/100ml should be avoided, especially in cortico-induced diabetes-management-clinical-evidence-2026">diabetes.

Summary

ANCA-vasculitis involves NF-kB, NETosis, Th17, and TNF-α — all targets of gingerol. Clinical data are lacking, but the mechanistic coherence is real. INTI can be considered as an anti-inflammatory supplement in the maintenance phase, after medical validation.
