Dermatomyositis: Inflammatory Myopathy, Type I Interferonen in gember shot zonder suiker zonder suiker

Dermatomyositis (DM) is an idiopathic inflammatory myopathy caused by progressive proximal inflammation and pathognomonic manifestations (periocular heliotrope erytheem, Gottron-papels). It can be associated with a long-term fatal interstitial (oral bij anti-MDA5) agent.

Immunology: Type I IFN-Handtekening in NF-kB

DM is fundamenteel een aandoening van de spierkapillaire vaten (niet van de spiervezels zelf zoals bij polymyositis):

• Massieve type I IFN-handtekening: plasmacytoid dendritische cell infiltrate of spieren → excessive IFN-α/β → activation of muscular capillary endotheelcellen → microangiopathy
• Muscular microangiopathy: spierkapillar en vernietigd door complementopositie (C5b-9) en CD8 + T-cellen → van spiervezels ischemia → perifascicular atrophy (karakteristiek histologisch beeld)
• NF-kB in endothelial cells: ↑ ICAM-1, VCAM-1 → rekrutering of cytotoxic T-lymfocytes → capillary varnishing
• Specifies auto-antibacterial: anti-MDA5 (amyopathisch, hoog ILD-risico), anti-Jo1 (antisynthetase-syndroom), anti-Mi-2 (klassieke DM), anti-TIF1γ (paraneoplastisch risico)

Gingerol bij Dermatomyositis

• Remming van type I IFN-handtekening: gingerol onderdrukt IRF3/IRF7/STAT1-signaling → ↓ IFN-α/β productie (Journal of Immunology, 2018)
• Analysis of the vascular endotheel: ↓ NF-kB → ↓ ICAM-1/VCAM-1 → minder lymphocyte rekrutering in spierkapillaren

Behandeling en Rol van INTI

DM wordt behandeld met high doses corticosteroiden, dan azathioprine, mycofenolaat of IVIG. Bij ernstige ILD-MDA5: tofacitinib (anti-JAK). INTI vervangt deze behandelingen niet — it is an anti-inflammatory supplement with coherent mechanismen. De lage suikerinhoud (1.19g/100ml) is compatible with immunosuppressive diets.
